Bridging anticoagulation remains a nuanced decision. Recent data suggests many patients can safely avoid bridging.

When Bridging MAY Be Needed:

  • Mechanical mitral valve
  • Recent stroke/TIA (<3 months)
  • Recent venous thromboembolism (<3 months)
  • Thrombophilic disorder (high-risk)

When Bridging is Likely UNNECESSARY:

  • Mechanical aortic valve (without risk factors)
  • Atrial fibrillation (CHADSβ‚‚-VASc ≀4)
  • VTE >3 months ago with low recurrence risk
  • Isolated proximal DVT/PE with transient risk factor

Bridge Regimen (if needed):

  • Therapeutic-dose LMWH (enoxaparin 1mg/kg BID) until INR therapeutic
  • Stop LMWH 24h pre-op
  • Restart warfarin POST-op as soon as hemostasis secure
  • Overlap with LMWH until INR therapeutic for 2 doses

Patient Communication:

  4. Plan: "Let's try this approach for 30 days and reevaluate"
