Quick Reference — Nutrition Screening Checklist

Use this checklist to identify patients who may benefit from nutrition intervention. ≥2 positive flags warrants dietitian referral.

  • 1Unintentional weight loss >5% in 1 month or >10% in 6 months
  • 2BMI <18.5 kg/m²
  • 3Reduced dietary intake for >7 days
  • 4Disease burden/nutrition impact (cancer, COPD, CKD, HF, GI disease)
  • 5Functional decline or frailty (slow gait, weak grip)
  • 6Serum albumin <3.5 g/dL or prealbumin <16 mg/dL
  • 7Edema or ascites masking weight changes
  • 8Polypharmacy (≥5 medications) affecting appetite or absorption
  • 9Social determinants: food insecurity, limited access, living alone
  • 10Recent hospitalization or surgical procedure
